Mecobalamin is the neurologically active form of vitamin B12 and occurs as a water-soluble vitamin in the body. It is a cofactor in the enzyme methionine synthase, which functions to transfer methyl groups for the regeneration of methionine from homocysteine. In anaemia, it increases erythrocyte production by promoting nucleic acid synthesis in the bone marrow and by promoting maturation and division of erythrocytes.

Silymarin possess hepatoprotective and antioxidant activity. The hepatoprotective effect is due to stimulation of synthesis of structural and functional proteins and phospholipids, as well as acceleration of the regeneration of hepatocytes.

Antioxidant effect is determined by interaction of bioflavones with free radicals in the liver and its detoxication. In such manner the process of peroxidation of the lipids is interupted and further liver destruction is prevented.

Clinically, these effects are manifested by improvement of the signs and symptoms and normalization of the liver variables (serum level of transaminases, gamma-globulins, and bilirubin).

For the treatment of jaundice, chronic inflammatory liver conditions, i.e. hepatitis, alcoholic liver damage and hepatic cirrhosis. It has anti-oxidant property.

Dosage

Siliba B dosage

Tablet: The usual adult dosage is one 500 mcg tablet three times daily. The dosage should be adjusted according to the age of patient and the severity of symptoms.

Injection:

  • Peripheral neuropathies: The usual adult dosage is one ampoule equivalent to 500 mcg of Mecobalamin, administered intramuscularly or intravenously three times a week.The dosage should be adjusted according to the age of patient and the severity of symptoms.
  • Megaloblastic anemia: The usual adult dosage is one ampoule equivalent to 500 mcg of Mecobalamin, administered intramuscularly or intravenously three times a week. After about two months of administration, dosage should be changed to one ampoule equivalent to 500 mcg of Mecobalamin every one to three months as maintenance therapy

70 mg or 140 mg or 500 mg capsule should be taken 3 times daily as per the instruction of a physician. The medication should be continued until the relief of the symptoms according to the advice of a physician.

Side Effects

Generally Mecobalamin is well tolerated. However, a few side effects like GI discomfort (including anorexia, nausea or diarrhea) & rash may be seen after administration of Mecobalamin.

A mild laxative effect has occasionally been observed.

Precaution

The medicine should not be used for months if there is no response at all after its use for a certain period of time.

Interaction

Decreased GI tract absorption with neomycin, aminosalicylic acid, H2-blockers and colchicine. Reduced serum concentrations with oral contraceptives. Reduced effects in anaemia with parenteral chloramphenicol.

Pregnancy & Breastfeeding use

Not recommended during pregnancy & lactation.

No experience is available about the use of Silymarin 70 mg or 140 mg during pregnancy and lactation. Therefore, if needed Silymarin 70 mg or 140 mg should be taken with caution according to the physician’s advice.

Contraindication

Hypersensitivity to any component of this product.

No adequate data of investigations are available about the use of this drug in children. Therefore, it should be used in children under 12 years of age with caution under the direct supervision of a physician.

Special Warning

Use in children: Not recommended.

Storage Condition

Oral: Store at room temperature. Protect from moisture and light.

Parenteral: Store at room temperature. Do not expose to direct light.

To be stored in a cool and dry place, away from direct sunlight. Keep the medicine out of reach of children.
